Objective

To conduct CERT Refresher, Theory & Practical Assessments for the CERT (Company Emergency Response Team, which includes elevating their skillsets and increase their confidence to tackle on-site incidents.

Who Should Attend

SMC members who had attended WSQ Implement Incident Management Processes course.

SIC members who had attended WSQ Implement Incident Management Processes and 3-day WSQ Response to Fire & Hazmat Emergency in Industries courses.

ERT members who attended 3-day WSQ Response to Fire & Hazmat Emergency in Industries and CERT First Aid courses.

Health Declaration Forms

The participant is required to fill-up a Self-Assessment Checklist and Fitness
Declaration Form (see Form A below). This form must be submitted to COSEM 2 weeks before the course commencement date. Please download the forms below:

• Form A- (Self Assessment Checklist & Fitness Declaration)
• Form-B (Information for doctors certifying fitness for participants)
• Form-C (Certificate of Fitness)
